Full Job Description

We are seeking a passionate and dedicated Pharmacist to join our admissions team at Morriston Hospital. In this pivotal role, you will be a key member of the multidisciplinary team optimising medication therapy for patients. You will ensure seamless transitions of care and enhance treatment outcomes from the moment they enter Morriston hospital.

As a key member of the admissions team, you will conduct comprehensive medication reviews, support prescribing decisions, and ensure safe and effective medication management for patients. You will collaborate closely with healthcare professionals to address medication-related issues promptly, helping to improve patient safety and outcomes.

You will also have opportunities to lead projects aimed at enhancing the efficiency of the admissions process and contribute to clinical guidelines and policies. Responsibilities include managing the pharmacy service in the admissions unit, undertaking medication reconciliation, overseeing medicines expenditure, and ensuring compliance with best practices and hospital protocols.

Main duties of the job

The post holder will be responsible for the day-to-day leadership and management of the pharmacy service to the medical admissions unit in Morriston Hospital.

The role will involve working collaboratively with pharmacy colleagues, clinicians, nursing staff and other members of the multi-disciplinary team to ensure the delivery of high-quality pharmaceutical care to newly admitted medical patients. The role will be mainly ward-based, providing and supervising a range of medicines management services for patients on the admissions unit.

The role will also involve identifying areas for improvement in pharmaceutical services within the admissions areas and implement strategies to enhance efficiency, quality, and patient satisfaction.

A major component will be the proactive provision of timely, evidence-based advice to prescribers to enhance patient care and promote cost-effective use of medicines. Supervision, mentoring and training of junior colleagues will also be a major component of the job.

The post holder will participate in dispensary duties and provide clinical leadership to junior pharmacists and trainees. In addition, the role will require the ongoing completion of core directorate activities including analysis of medicines expenditure, clinical governance audits, service improvement projects and risk management.

The ability to speak Welsh is desirable for this post; both English and Welsh speakers are encouraged to apply.
