Full Job Description

Are you looking for a new challenge and passionate about the area of Elderly Care?, You will lead on all aspects of medicines optimisation and medication use within the specialism of elderly care, supporting medical and nursing teams, as well as allied health professionals in the safe and effective use of medicines.

Pharmacy and Medicines Optimisation at the Trust are well developed and we are committed to sharing excellence through our clinical services and teaching roles. The department is committed to the use of new technology using ASCribe/EMIS, Omnicell electronic storage cabinets, ChemoCare and TrakCare electronic prescribing systems and two state of the art BD Rowa Vmax robots.

We will support you to continue to develop your advanced skills in clinical pharmacy, and would expect you to have a significant impact on the use of medicines by supporting safe and high quality services and supporting the patient journey., Working within and leading a small pharmacy team, you will use appropriate skill mix within the team to ensure that medicines reconciliations are completed in a timely manner, in line with current national recommendations, safe and efficient use of patient's own medication and effective communication through transfers of care.

Through excellent leadership skills, you will be expected to implement changes required to ensure that the service runs in a safe, efficient and cost-effective way.
