Full Job Description

Our community Nursing and Therapy teams are currently undergoing a transformation programme as we continue to grow Urgent Crisis Response and NHS@Home in Wiltshire

Our Vision is to integrate our core services, so we have a range of skills to not only support and enable those with long term conditions including Frailty but to also to assess and treat those who are acutely unwell so they can remain in the place they call home.

We are looking for Advanced Practitioners (Nurses, Paramedics, Physiotherapists, and Occupational Therapists) or those who are on a development pathway to come and join our teams. We want people to apply who are confident to assess the deteriorating person and work with a multi-professional team to support recovery at home. This service has a wide variety of caseload where no day is the same and this role will suit those who like to think outside of the box be creative and work closely with our partners in health, care and the voluntary sector.

This is a great opportunity to develop expert clinical skills in frailty, respiratory, Heart Failure and rehabilitation and a unique opening to be part of service redesign using new technology and quality improvement methodology to learn, evaluate, adapt and implement.

The service runs 8-8, 7 days a week and you will work a shift pattern which includes mornings, evenings, weekends, and public holidays.

Wiltshire Health and Care is a community provider which is known for investing in staff, supporting career development and embracing change to ensure our services are able to respond to our communitys health needs. We are the bridge between our Acute partners and community health and care.

We currently employ ACPs across Community services, MSK Out-patients, Heart failure, Respiratory, and In-patients. You will be part of a large peer group and have opportunities to learn together and consider specialising. This is very much a patient facing role and you will be travelling between bases and peoples homes/ care homes. If you have not experienced community care before you, we will give you a full induction.

Working in the community is both rewarding and a privilege; this is where you truly get to understand the many ways that people chose to live their lives and understand the range of health inequalities which sadly exist across our county. Travel time between visits allows you to reflect on your caseload and to work with colleagues and students; teaching and mentoring as you go. We welcome and encourage people to apply who have diverse profiles and experiences; we embrace all learning opportunities to improve.

The successful candidates will be based in one of our three localities, North, South or West Wiltshire., Wiltshire Health and Care provides community based health services, enabling people to live healthy independent lives. Our Vision is to enable people to live independant and fulfilling lives for as long as possible. We are an NHS partnership organisation, focused entirely on delivering quality health and care to the people of Wiltshire - not on making a profit.

There has never been a better time to join us. Community services are expanding as part of the NHS Long Term Plan and the population of Wiltshire has never needed the energy, enthusiasm and commitment of our community-focused teams more than now. You will join a friendly and supportive team who live our values of Quality, Integrity, Partnership and Change, which were developed with our staff, patients and carers.

If you already live in or near Wiltshire, youll know that it is a great place to live and work: rolling countryside, bustling market towns and vibrant city life combined with excellent transport links and good schools. If you are looking to move to the area, we offer local employment and a chance to play a crucial role as part of your new community straight away. What are you waiting for?!
