Full Job Description

An exciting opportunity has arisen to work within the Gateshead Central South Primary Care Network (PCN). We are looking for 3 Advanced Clinical Practitioners to join our well-established PCN which is comprised of 8 GP practices.Each practitioner would work across the PCN, however there may be some central hub working and also the need for cross cover during holiday times.The job will be patient facing, but may include telephone triage and consultations to allow greater patient access to services., The role of the Advance Clinical Practitioner includes the following: Minor illness (essential) Management of minor ailments Chronic Disease Review ECGs Chaperoning and assisting patients where appropriate who are being examined by another clinician Participation in administrative systems in the Practice

  • Maintenance of accurate and timely clinical records
  • Maintaining a tidy and clean environment in the treatment room in line with infection control requirements
  • Attend and participate in practice/PCN meetings when required
  • Any other delegated duties appropriate to the post

    Applicants must have the necessary Advanced Clinical Practitioner qualification and must be an independent prescriber.

    The PCN practices cover much of the Central and South Gateshead population, providing care to over 60,000 patients with diverse health care needs. Each practice runs independently, with an individual practice team but all practices work together on shared ethos of best possible patient care and joint goals for the PCN. You would be joining an already well established team of Additional Roles staff including Pharmacists, Musculoskeletal First Contact Practitioners, Mental Health Practitioner, Counsellors and Care Coordinators.
