Full Job Description

We are looking for an Advanced Clinical Practitioner to work within ESNEFT's Urgent Treatment service in Clacton & Harwich, providing a holistic approach to the care of individuals.
You will provide professional leadership, clinical expertise and manage same day presentations of patients to ESNEFT Urgent Treatment Service
1 x Permanent role
1 x Fixed term role to cease 30th September 2025 (This will be on a secondment basis for an internal applicant)
As a lead nursing clinician of this service you will be expected to work in partnership with primary and secondary care colleagues and other agencies to secure a smooth patient journey across the whole patient pathway. Self-care will be encouraged, and patients and carers will be actively involved in the development of their care plan and the management of their condition.
You will also act as a role model while promoting clinical effectiveness and delivering advanced practice. .You will be expected to perform comprehensive assessment of patients with same-day urgent medical, diverse minor injury & minor illness and urgent presentations and develop working diagnosis and treatment plans whilst working without direct supervision, with virtual input from the medical team., + You will have responsibility for the organisation's delivery of nursing care within ESNEFT's Urgent Treatment Centres, including ensuring appropriate staffing levels/skill mix.
+ You are required to undertake all aspects of clinical duties as an autonomous practitioner
+ To lead for MIU specific changes to practice or service protocols in order to improve quality of care to patients.
+ Identify the need for audit, evaluation and research projects, co-ordinating their implementation
+ To deal with complaints relating to the clinical team.

+ Diagnose conditions, selecting and interpreting diagnostic procedures, using clinical reasoning to formulate the care plan
+ Develop specialised care plans, identifying who will carry out each aspect and delegating tasks to more junior staff, assistants and volunteers
+ Provide specialist advice to patients, parents carers and the wider MDT
+ To continuously evaluate and manage patient progress during treatment and alter accordingly to achieve ongoing goals in collaboration with the patient.
+ Provide advanced case management for complex cases.
