Advanced Clinical Practitioner

You will support core general practice capacity and work as part of a positive, proactive and collaborative team. You will be experienced in general practice and be able to work autonomously, managing a caseload of booked appointments for on-the-day and urgent care needs, with minimal indirect supervision from the onsite GP under a primary care consultant model. You will need to be skilled in providing urgent care and minor illness management and providing support to patients with long term conditions.

We are a team that is dedicated to delivering exceptional high quality care to our patients. We have a diverse practice team including the additional roles of social prescribing link workers, Clinical Pharmacist, Care Coordinator and Paramedics. We are active members of our Primary Care Network.

The position available is for 20 hours per week,across 3 days - Tuesday, Wednesday and Friday.

The hourly rate for this role is from £22.00 - £33.00, depending on experience
