Full Job Description

You will be able to expertly manage urgent community referrals, from our GPs, Hospital and Home and community nurse colleagues. You will manage these cohort(s) of patients and presenting complaints, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care, demonstrating safe, competent clinical decision-making and expert care evidence-based care.

To be a clinical expert and senior clinician in the field of Adult Community Care. To work with the senior leadership, ACPs and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for our patients.

The successful applicant will need to share the values of Mid Yorkshire Teaching Trust and work as part of a large multi-disciplinary team including GPs, Hospital specialty teams, Community Nurses, Clinical Pharmacists, Allied Health Professionals and Administrators.

You will be passionate about making a difference for people, and working with them in the communities where they live. You will need to have an exceptional understanding of the positive impact of effective teamwork across the Division and organisation.

You will bring your clinical expertise to a wider multidisciplinary team. You will be an experienced, registered health and care practitioner and have a high degree of autonomy and complex decision-making skills. This is underpinned by a masters level that encompasses the four pillars of clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area specific clinical competence. Clinical support and supervision are available for all staff.

Advanced clinical practice embodies the ability to manage clinical care in partnership with individuals, families, and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance peoples experience and improve outcomes.
