The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. During the COVID-19 pandemic, it has been necessary to take significant steps to protect the health and safety of our staff, service users and those attending our sites. Unless exempt, we encourage and support staff to be vaccinated as this remains the best line of defence against COVID-19., You will work alongside our GPs & ACPs assessing & managing patient's presenting to our Same Day Emergency Care unit (SDEC) & who have been admitted on Cornwall's Virtual Ward.
SDEC provides an alternative to the acute hospital for patients who can be safely managed outside of the emergency department with the aim of home on the same day. The follow up to this, is provided by our Virtual Ward. Patients are kept at home with regular nursing contact & medical oversight 7 days a week. This is a Consultant-led service with opportunities to join ward rounds & MDTs for learning & service-provision.
We also host a Urgent Treatment Centre &Community Assessment and Treatment Unit (CATU). The post holder may rotate through these services as well depending on skill & service requirement. The UTC sees a range of medical problems covering all age groups, self-presenting to the unit. The CATU provides short-stay, ward-based medical and nursing care, with therapy assessments.
The role will be varied and challenging but well-supported with our existing GPs. There is time and scope for leadership, personal development and project opportunities. The role will can have an element of bespoke creation, while supporting us to deliver safe provision of medical cover to our patients across these services.
This role will suit an experienced ACP, comfortable with working independently., History-taking, examination, requesting investigations and creating an appropriate safe medical management plan for patients presenting to the services above.
Request and interpret investigations such as blood tests and x-rays.
Prescribe appropriate medications to support the patient's care.
Utilise all alternatives to admission services to enable people to be treated in the own homes whenever possible.
Work closely and collaboratively with your team of the day, with a 'can-do' 'yes' approach to your working.
