Full Job Description
The post holder will use advance assessment and diagnostic skills to assess and treat patients. The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community and hospital settings as appropriate. The post holder will:
- Be a Qualified Advanced Nurse Practitioner
- Act as a role model
- Be responsible for undertaking comprehensive health assessments, physical examination and ordering tests for patients in Rapid Response
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population.
- Be involved in establishing delivering and leading a comprehensive service within community services
- You will play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and through networking increase the number of referrals from the acute and community.

We provide an Emotional Wellbeing Mental Health Service for children and young people across the whole of Essex. We are the provider of all age eating disorder services and child and adolescent mental health services across Kent and Medway. With an annual budget of £355 million, we provide care and treatment for a population of circa 4.3 million. We employ approximately 6,000 staff who work across 210 bases in London, Essex, Kent and Medway. An exciting role for an Advanced Clinical Practitioner has been created to join the dynamic CTT team that covers Barking & Dagenham, Havering, & Redbridge. Community Treatment Team (CTT) aim to avoid unnecessary admissions to the acute hospital settings by managing patients in crisis in their own homes. The post holder will deliver high quality holistic nursing as part of a multidisciplinary team within the accident and emergency and community setting, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735).
