Full Job Description

+ Haematology at St Georges has recently been approved funding via a nationally funded initiative to provide streamlined care to people with sickle cell disease.
+ This is a two year project, delivered through the National Healthcare Inequalities Improvement Programme.
+ The aim is ultimately to improve access, experience, outcomes, and quality of life for or patients with Sickle Cell disease.
+ The project will provide a dedicated pathway for our patients including the set-up of an emergency department bypass.
+ We are looking for an experienced ACP who will become part of a wider team of consultants, clinical nurse specialists, pharmacists and AHP in our Haemoglobinopathy service.

+ Informal visits are strongly recommended and we would be happy to talk through the plans for the development of the pathway., + The post holder will be a registered nurse practicing autonomously as an Advanced Practitioner (AP) to provide patient-centred quality care.
+ This will encompass the skills of advanced assessment, examination, diagnosis and management within an agreed scope of practice.
+ The AP will meet the requirements of the Centre for Advancing Practice's framework and guidelines for advanced level practice.
+ To work within the multi-professional framework for advanced clinical practice in England (HEE 2017).
+ The post holder will deliver an 80% clinical component to their role and 20% related to leadership and management, education and research.
+ To provide excellent care and a high-quality service for Haemoglobinopathy patients within St George's Hospital NHS Foundation with a variety of red cell related disorders including Sickle Cell Disease and Thalassaemia.
+ To support with the Hyper Acute Unit in-hours triage function, providing continuous in-hours cover.
+ To support with the wider training and education of Ward and Haematology junior doctor workforce.
+ To engage with other trusts within the network to ensure local providers are aware of the resources available within Haemoglobinopathy.
+ To contribute to the ongoing development of clinical practice, research and standards of care within Haemoglobinopathy.
+ The post holder will undertake the development of adult haemoglobinopathies educational resources. Therefore, adult haemoglobinopathy experience would be desirable.
