More detail about the role: This is an exciting new post and the start of a development whereby we aim to enhance inpatient care for frail older people, exploring new ways of working within the multi-disciplinary team to enhance inpatient care and discharge from hospital of patients on acute geriatric wards. The postholder will be based on a 24-bedded ward for the acute and post-acute care of frail older people with complex co-morbidities, dementia and delirium. The postholder would share care of post-acute patients with the medical Consultants, focussing on progressing complex geriatric assessment and ensuring that the ongoing management for the patient is focussed on their goals, whilst effecting safe, timely, well-planned discharge from hospital. We aim that introduction of this new way of working will free up Consultant Geriatrician time to deliver other Consultant-led activities
