Full Job Description
We have an exciting opportunity for an ACP to join our Integrated Heart Failure Service at Liverpool University Hospital Foundation NHS Trust. We are looking for an ACP to support our Virtual Ward Urgent Response Service.
The service will operate 7 days per week8 am till 8 pm and requires flexibility working within an evolving proactive, dynamic and committed multidisciplinary Heart failure Team
Using Tele medicine, the candidate will have clinical oversight and (in conjunction with Consultants) responsibility for patients who are being managed in their own homes. This will involve working with external agencies and technologies to provide an enhanced Hospital at Home and Early Supported Discharge service for patients with Heart Failure.
The successful candidatewill need to demonstrate strong clinical reasoning and prioritisation skills be an effective leader and have the ability to work independently and as a member of a team., 1.To improve the experience and outcomes of care for patients by taking thelead in investigations and treatments prescribed.2. To provide a consistent and personalised service by ensuring that thepatients receive the best care, in the best place from the best person or team.3. To provide social, emotional and psychological support for patients fromreferral and throughout their treatment.4. To deliver expert specialist evidence-based care including development,assessment, planning, implementing, evaluating programmes of evidencebased care for a defined caseload.5. To make appropriate referral to consultants/other specialist/members of themultidisciplinary team in primary, secondary or tertiary care settings.6. To be aware of risk management issues both locally and nationally anddevelop strategies to address identified issues.
