Full Job Description

You will primarily work as part of our medical team, responsible for the clinical care of our 30 patients.
Patients are admitted to Skylark Rehabilitation Unit for rehabilitation and recovery following an acute hospital admission.
Throughout their admission, patients require medical intervention to:
+ Continue the management of their health needs and underlying conditions.
+ Optimise the patient medically to maximise their rehabilitation potential.
+ Utilise the multidisciplinary workforce to enhance the patients overall health.
+ React and provide urgent medical intervention to manage new medical deterioration.
You will be responsible for the ongoing management of the patient's current cause of admission and other co-morbidities and supporting the wider multi-disciplinary workforce in delivering safe and effective care including rehabilitation of the patient to allow their recovery and return home.
+ Assessment and clerking of new patients admitted onto the unit.
+ Writing drug charts to allow safe administration of medications.
+ Noting and acting upon important and outstanding medical events from the patient's admission into secondary care.
+ Working with collaboratively with colleagues at our acute partner hospitals and our sytsem partners.

We are seeking an advanced clinical practitioner to join our stroke medical workforce including GP's with special interest in frailty and Advanced Practitioner.
The unit consists of 30 beds based at Skylark Rehab Unit in Yate, which provides an environment where individuals who are medically stable can receive rehabilitation assessment. The multi-disciplinary team works collaboratively to achieve safe, effective care for our patients whilst they aim to recover from hospital admissions and return home.

You will be supported in applying your experience and knowledge in an encouraging environment aiming to enhance your skills in both urgent care and other long-term conditions.
