As an ACP, you will work within the Four Pillars of Advanced Practice to provide exceptional care in both face-to-face and remote settings, collaborating with a multi-disciplinary team and Integrated Care Board (ICB). You will help manage patients with long-term conditions, co-morbidities, and poly-pharmacy, driving improvements in clinical effectiveness and safety. Your contributions will directly impact the quality of care we provide to our community. This role requires a blend of clinical expertise, critical thinking, and a proactive approach to patient care. - Conduct comprehensive patient assessments, including history taking and physical examinations.
- Develop, implement, and evaluate individualised care plans, with a primary focus on medicines management.
- Prescribe and manage medications in line with clinical guidelines and best practices.
- Monitor and evaluate the therapeutic effects of medications, adjusting treatment plans as needed.
- Provide expert advice on the safe and effective use of medicines to patients, families, and healthcare professionals. Medicines Management:
- Lead and coordinate initiatives to optimise the use of medicines, minimise medication errors, and improve patient safety.
- Develop and implement policies and procedures related to medicines management.
- Conduct clinical audits and participate in quality improvement projects to enhance practices.
- Collaborate with pharmacy, nursing, and medical staff to deliver comprehensive medication management. Education and Training:
- Educate healthcare staff on medicines management and pharmacotherapy to foster best practices.
- Mentor and support junior staff and students, contributing to a culture of continuous learning and development. Leadership and Collaboration:
- Participate in multidisciplinary team meetings and contribute to the development of clinical guidelines and pathways.
- Represent the organisation at local, regional, and national forums related to medicines management.
- Act as a resource and advocate for patients, families, and staff on medication-related issues.
