Full-time & Part-time Advanced Clinical Practitioner £54,000 to £65,250, yearly salary increments, NHS Pension provider. gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have fantastic opportunities for Advanced Clinical Practitioners to work in our Primary Care GP Practice services. We operate 13 GP practices across Central Manchester, North Manchester and Tameside & Glossop, due to continued growth of our primary care services, we are looking to employ additional Advanced Clinical Practitioners ACP/ANP. Based at an induvial GP Practice or multisite portfolio role across a number of GP Practices, if desired, with flexible full-time and part-time hours available for Advanced Clinical Practitioners. Whilst working as an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team that includes Nursing, Allied Health Professionals and GPs, who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care. We are a values driven organisation and we are passionate about providing the best possible healthcare for our patients., Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). You will be supported by the non clinical team who have a good knowledge of our extensive local hospital and community services. The clinical team includes GP's, Practice Nurse, Pharmacist, Paramedic and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention. In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC. You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP. The extensive footprint of the organisation provides you with an opportunity to be exposed to a wide variety of different services such as urgent care, specialist services and out of hours working, where shared learning is encouraged.
