An exciting opportunity has arisen for a mutlidisciplinary Advanced Clinical Practitioner (ACP) (NMC/HCPC/GPhC) at Band 8a to join our Urgent Community Response Services Directorate within the Primary Care, Community, and Therapies Clinical Group. You will be supported by our Lead ACP, who works closely with the Director of Advanced Practice for the organisation, ensuring appropriate supervision, governance and fulfilling all four pillars of ACP. As part of this dynamic team, you'll collaborate with ACPs, trainee ACPs (tACPs), enhanced practitioners, community matrons, and case managers. Your role will primarily focus on the UCR directorate, but you'll also work across Primary Care and broader community services--key elements of our Trust's commitment to delivering excellent and responsive integrated care.   • Clinical Leadership:
  • + Provide clinical leadership and support for trainee ACPs (tACPs) and the wider UCR workforce across the Primary Care, Community, and Therapies Clinical Group. + Play a key role in the development and supervision of advanced practice within the team.
  • Service Development:
  • + Contribute to the ongoing development of UCR services, governance processes, and the establishment of high clinical standards.
  • Patient Care:
  • + Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions, where appropriate. + Diagnose and manage both acute and chronic conditions, integrating drug-based and non-drug-based treatment methods into comprehensive management plans. + Prescribe and review medication to ensure therapeutic effectiveness, aligned with patient needs, evidence-based practice, national guidelines, and formulary protocols.
