Your role will primarily focus on the UCR directorate, but you'll also work across Primary Care and broader community services--key elements of our Trust's commitment to delivering excellent and responsive integrated care. Why Join Us? Our Trust is proud to be the first in the Midlands to develop a true Care Navigation Centre, which supports the local population in accessing emergency care, urgent and non-urgent community services. This innovative service is delivered by a multidisciplinary team (MDT) of clinical and administrative professionals, embracing a "no wrong door" ethos and ensuring the right patient reaches the right team, first time.,

- Patient Care: + Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions, where appropriate. + Diagnose and manage both acute and chronic conditions, integrating drug-based and non-drug-based treatment methods into comprehensive management plans. + Prescribe and review medication to ensure therapeutic effectiveness, aligned with patient needs, evidence-based practice, national guidelines, and formulary protocols.
If you're an experienced, forward-thinking practitioner, passionate about innovation and integrated care, we would love to hear from you., The successful candidate will be an autonomous practitioner, conducting independent health assessments for undifferentiated and undiagnosed conditions, specifically focusing on the nine Urgent Community Response (UCR) conditions.
Sandwell and West Birmingham NHS Trust (SWBT) is an integrated care organisation with a budget of approximately £600 million and over 7,000 staff. Diversity and social care is at the core of what we do as the Trust provides Community and Acute Services to over half a million people in an urban centre that demands massive regeneration and has substantial premature mortality. Our new acute hospital, the Midland Metropolitan University Hospital (MMUH), opens in 2024 and will provide care to our local population from first class, purpose-built premises. The development of the new hospital will play an important role in the regeneration of the wider area and in improving the lives of local people and reducing health inequalities. We have three newly emerging strategic objectives: 1. Our People - to cultivate and sustain happy, productive and engaged staff 2. Our Patients - to be good or outstanding in everything we do 3. Our Population - to work seamlessly with our partners to improve lives
