Advanced Clinical Practitioner - WCC PCN

We are seeking an experienced Advanced Clinical Practitioner (ACP) to join our Acute Visiting Service. In this role, you will work across multiple sites within Worcester City PCN, providing expert clinical care to patients, including those who are frail or housebound. You will be an integral part of our multidisciplinary team, supporting the delivery of high-standard healthcare services., This is a fantastic opportunity to play a key role in a dynamic and supportive team. You will have the chance to make a significant impact on patient care within the community, working with a diverse and complex patient population.
