We have an exciting opportunity for an Advanced Mental Health Practitioner to join our vibrant, multi-disciplinary Adults Community Mental Health Team in Slough. Our team is a well-established group of dedicated professionals, including social workers, community mental health nurses, consultant psychiatrists, psychological therapists, occupational therapists, community support workers, employment specialists, and administrative staff. Jointly funded by the Berkshire Healthcare NHS Foundation Trust and Slough Borough Council, we are committed to providing top-class care and support to adults with significant mental illnesses and their carers. Your Role: As an Advanced Mental Health Practitioner, you'll: Provide Clinical Leadership: Offer continued clinical advice, training, and supervision to staff within our multi-disciplinary team. Coordinate Care: Liaise with local services to ensure smooth transitions and effective support for our clients. Manage Caseloads: Handle your own caseload of clients with complex mental health needs, conducting risk assessments and formulating care plans. Collaborate: Work closely with service users, their families, and carers to ensure the best possible outcomes.
