It involves providing comprehensive care to patients of all ages and backgrounds, addressing both acute medical needs and long-term health management.   • Monitor, review and evaluate care interventions to ensure optimum quality is maintained.
  • Lead on the assessment, diagnosis, planning, implementation and evaluation of treatment / interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions / treatments for patients with complex needs.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non - drug based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and SHFT protocols, and within scope of practice.
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Inform clinical manager of any concerns regarding patients, relatives, visitors or staff that may compromise patients care.
  • The role will involve, but not be limited to you performing the above.

  • Registered First Level Nurse.
  • Relevant nursing / health degree.
  • History taking and physical assessment course.
  • Clinical Supervision Training.
  • Independent and supplementary Nurse prescribing course.
  • Teaching and Assessing qualification ENB 998 or equivalent / mentorship qualification PETALS.
  • Post registration degree or Masters Degree.
  • Management / leadership qualification / development programme.
  • Nurse practitioner qualification.
  • Minor illness course.
  • What youll need :
  • A minimum of 6 months paid experience in an Acute hospital as Advanced Nurse's ward environment within the last 3 years
  • A minimum of 4 years post registration experience, of which at least two have been in a senior role.
  • High level of clinical skill in particular recognising the deteriorating patient, and the management of patients suffering exacerbation of long term conditions such as COPD.
  • Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary / community care setting with the additional awareness of referral pathways into the wider multidisciplinary team.
  • Work autonomously within own competencies.
  • Experience of initiating or participating in clinical audit / research relating to clinical practice / service provision.
  • Participates in regular clinical supervision.
  • Experience of caseload management including responsibility for complex care packages for vulnerable people.
  • Involved in the implementation and management of change.
  • Depending on the Trust, you may require variable additional certification and skills., Statement of NMC Registration
  • Degree / Diploma in Nursing
  • Experience Essential
