Full Job Description

The post available is to support our housebound patients within their own homes who are registered with one of our 9 General Practices, however you will also be required to work within a GP Practice at times. This role may include extended hours and access care provision., To deliver advanced level practice which will encompass aspects of education, research and management but is firmly grounded in direct compassionate care provision or clinical work with patients, families, caseloads, and populations. Nurses working at an advanced level will promote public health and well-being using 'making every contact count' as an approach to change behaviour through interactions with individuals, carers and families to support them in making positive changes to their physical and mental health and wellbeing.
80% role clinical
20% role in education/research/service development.

We are looking for an experienced Advanced Nurse or Clinical Practitioner who must have been qualified for a minimum of 3 years with 1 year recent experience within general practice or minor illness care provision. In return you will receive a robust support system by our dedicated multidisciplinary team across the Primary Care Network.
You must have a full UK driving licence and be a car owner.
