Are you an Advanced Nurse Practitioner and non-medical prescriber (V300)? If not are you a Adult trained nurse with community/primary care/urgent care experience who has been working at an advanced level autonomously that is looking to develop their skills, then this is the role for you!

We are looking for an adult trained nurse working at an advanced level who is looking for an new challenge working alongside our care navigator and clinical operational lead to support our online consultations and our new neighbourhood team., This will be a new role supporting the development of our new neighbourhood team for the PCN. As a PCN we are required to carry out holistic assessments to cover all health, social and wellbeing aspects of the patients in care homes to develop personalised care and support plans. You will be required to identify the complex care needs of those frail patients and support the care home staff using risk stratification tools to identify this cohort of patients like the Rockwood score and electronic frailty index. You will visit care homes regularly building a rapport with the staff and patients to support the nursing team with assessment of the patients needs, liaise with the wider MDT's, do referrals to appropriate others as needed and liaise with the named GP as needed. This role will also provide holiday/sickness cover the electronic e-hub consultations online for the Practices. You will advise/prescribe using an electronic consultations system with EMIS to patients within a remit of
minor injuries and illness, full training will be given.

Rural rother PCN is based in an Office in Battle East Sussex. We have an excellent team of clinical pharmacists, physios, paramedic practitioners, social prescribers and care navigators that all provide a service to our patients in the rural rother area. Our surgery teams are working closely with each other, enjoying the ability to share expertise and resources, to develop new services.
