Full Job Description

We have an exciting opportunity for an experienced LD Nurse to join our dynamic and forward-thinking Adult Learning Disability Community Health Team in Barnsley.

The post is full time, 37.5 hours per week over five days.

The Community LD Health Team is team is looking for a highly motivated, committed, and enthusiastic Learning Disability Nurse whos passionate about delivering high quality compassionate care and can demonstrate safe clinical decision making. The post holder will be a highly skilled in the management of complex care and the health needs of people with Learning Disabilities.

The successful applicant will provide leadership and management for the nursing team whilst providing both proactive and responsive care to service users and their support networks., Leading the delivery of the nursing service within the overall locality clinical team.

Providing specialist nursing assessment and intervention with individuals professional expertise to an allocated caseload of more complex cases held by the team.

Utilising specialist and advanced knowledge of therapy models of practice to guide intervention and assessment methods appropriate to individual service user needs.

Working with service users to identify nursing goals as part of the overall care plan, using assessment tools based on current evidence based best practice.

Supporting the culture and implementation of service user led care plans to achieve therapeutic goals.

Ensuring the nursing team monitors, evaluates and modifies treatment in order to measure progress and ensure effectiveness of intervention.

Working with other lead Nurses, the post holder will provide trust wide leadership and support within their own discipline and the ability to bring new creative ideas to develop the service.
