Full Job Description

A unique and rare opportunity has arisen for an Advanced Nurse Practitioner to join the Learning Disability Service working across Northamptonshire to support primary care services and undertake Annual Health Checks for 14+ with a Learning Disability. The expansion of the community team is part of the national community transformation process evolving in line with the NHS Long Term Plan. This is an exciting opportunity to be at the heart of responding to national change and developments in the delivery of care for those with a Learning Disability., The post holder will combine high level assessment to make a diagnosis, initiate medical and/or nursing treatments, refer appropriately and lead the co-ordination of care working with partner agencies and key stakeholders. You will be expected to support colleagues and be responsible for non-medical prescribing, managing an age inclusive caseload, facilitate clinics and use a holistic clinical assessment approach. You will also manage the implementation of a range of care interventions for service users presenting with undifferentiated or undiagnosed problems.
