Advanced Nurse Practitioner

The Advanced Clinical Practitioner (ACP) plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and all demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The ACP operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners., Clinical Responsibilities:

Patient Care: Deliver advanced and complex clinical care

Autonomous Practice: Work independently within the scope of practice to manage complete and undifferentiated clinical episodes

Case Management: Manage patient cases from initial assessment to conclusion.

Prescribing: Utilise prescriptive authority responsibly

Home visiting: undertake home visiting as and when required

Leadership and Guidance:

Mentorship: Provide mentorship and supervision to nursing staff

Service Development: Lead the development and implementation of new clinical services

Professional Development: Engage in continuous professional development

Research and Quality Improvement:

Research Participation: Participate in clinical audits and research activities

Quality Assurance: Implement quality improvement initiatives

Educational Activities:

Training Provision: Deliver training sessions to healthcare staff

CPD Activities

Administrative and Organisational Duties:

Record Keeping: Maintain comprehensive records using SystmOne.

Meetings: Attend and contribute to team meetings

The 4 sites in Gateshead operate at Crawcrook, Blaydon, Grange Road and Rowlands Gill, providing care to over 20,500 patients.

The organisation is patient-driven and has used patient feedback in the past to develop innovative services such as a bespoke smart phone app, patient-centred practice websites, joint working with the third sector and a bespoke e-consultation service.
