Full Job Description

We are looking for a motivated and enthusiastic Advanced Nurse Practitioner to join our friendly team working in the town of New Mills in the beautiful High Peak, close to Stockport. We are a small practice with around 4600 patients, and we value the ability which this list size gives us to get to know our patients well as individuals. Our shared ethos allows us to practice a more traditional style of patient-focused family medicine with a good level of continuity of care. We pride ourselves on providing excellent care for our patients within our established team. We value teamwork, mutual support, and a friendly working environment and are looking for someone who shares these values and our way of working. We are looking for someone who will enjoy working as part of our multidisciplinary team and is also confident to work independently within their professional capabilities. We believe that this role would best suit someone who is already experienced in working independently in
General Practice. We are looking for someone to work part time, between 16 and 24 hours/week., The successful candidate will work alongside our multi-disciplinary team of GPs, practice nurse, HCAs and clinical pharmacist to provide care and treatment to patients with either long term conditions or undiagnosed undifferentiated diagnoses. You will work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness or injury across all age groups. You will also support and lead the practice nursing team.

A key part of the role will be taking on the role of Community Matron for our housebound frail and vulnerable patients. In this role you will work closely with the multi-disciplinary team and practice care co-ordinator to identify those patients most at risk and work pre-emptively to monitor their chronic conditions and identify their clinical needs to reduce the need for admission.

You will also lead on the care and monitoring of our patients in two residential homes for adults with learning disabilities, with the support of the GPs.

We also envisage a part of this role will include seeing patients in an acute clinic setting in the practice.

About us

We are an established practice with excellent team working, and our practice area covers New Mills, Hayfield, and the surrounding area, extending part way into Disley at one end and Whaley Bridge at the other. We use EMIS as our clinical system. We are fortunate to work from a large purpose-built premises in the centre of New Mills.

We are part of High Peak and Buxton PCN, made up of 8 practices from New Mills to Hartington. We are active members of this PCN and enjoy good working relationships with all our neighbouring practices

Our clinical team comprises 2 GP partners, 1 salaried GP, 1 practice nurse, 2 HCAs and 1 clinical pharmacist andsupported by the wider community and PCN multi-disciplinary team, the PCN pharmacy hub for general medication queries, First Contact Practitioners, Mental Health Worker and Social Prescribers. The PCN home visiting service is able to undertake most of our acute home visit requests. We have an excellent Practice Care Co-ordinator who chairs our weekly MDT meetings and our excellent District Nursing team with whom we have a close and collaborative working relationship.

We also have a weekly clinical meeting so all clinicians can informally discuss patients of concern and discuss any new developments within the practice.

We are a training practice who welcome GP specialist trainees as well as medical students.

We close once a month on a Wednesday afternoon for full team training. Some of these sessions are joint with our PCN.
