Full Job Description
To workas an autonomous practitioner, responsible for the delivery of practice nursingservices, working as part of the practice multidisciplinary team, deliveringcare within their scope of practice to the entitled patient population. The advancednurse practitioner will be responsible for a number of clinical areas such ashealth promotion, chronic disease management, health prevention, well women andwell man clinics, as well as supporting the management team in the reviewing ofclinical policy and procedure. Main duties of the job The following are thecore responsibilities of the advanced nurse may be onoccasion, a requirement to carry out other tasks, The following are the core responsibilities of the administrator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels : a. Receive correspondence from external agencies and process accordingly b. Scanning documents to the relevant software programme and logging them c. Filtering documents suitably according to need and passing to the relevant clinicians to read and action d. Initiating contact with and responding to, requests from patients, team members and external agencies e. Photocopy documentation as required f. Support administrative staff, providing cover during staff absences g. Input data into the patients healthcare records as necessary h. Manage all queries as necessary in an efficient manner i. Maintain a clean, tidy, effective working area at all times j. Support all clinical staff with general tasks as requested k. Completion of administrative tasks in relation to patient results and letters Secondary Responsibilities In addition to the primary responsibilities, the receptionist may be requested to : a. Maintaining and monitoring the practice appointment system b. Process personal, telephone and e-requests for appointments c. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately d. Signpost patients to the correct service e. Partake in audit as directed by the audit lead f. Action incoming faxes when necessary g. Scanning of patient related documentation and attaching scanned documents to patients healthcare records h. Complete opening and closing procedures in accordance with the duty rota,
- Wound Care / Removal of sutures & staples
- ECGs
- Venepuncture
- New patient medicals
- Chaperone procedure
- Requesting pathology tests and processing the results, advising patients accordingly
- Travel medicine
- Diabetes
- Hypertension
- Asthma
- Spirometry
- Immunisations (routine, childhood and travel)
- Womens health (Cervical cytology, contraception, etc.)
- Understands the importance of evidence based practice
- Broad knowledge of clinical governance
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understanding when to refer to GPs
- Knowledge of health promotion strategies
- Understands the requirement for PGDs and associated policy
Registered Nurse; - Post graduate diploma or degree (Advanced Practice Qualification);
- Qualified Nurse Prescriber;
- Qualified Triage Nurse; Desirable
- Minor Illness Qualification;
- Teaching Qualification;
- ALS, PALS; Experience Essential
- Experience working in a primary care environment;
- Experience in chronic disease management;
- Experience in prescribing and undertaking medication reviews; Desirable Experience working as a practice nurse or community nurse,
- Knowledge of public health issues in the local area
- Awareness of issues within the wider health arena
