Weare looking for enthusiastic and dynamic Advanced Nurse Practitioners to jointhe new innovative specialist Hospital@Home Frailty Team / Frailty AssessmentUnit. We are in the process ofdeveloping a new pathway which involves the identification and management offrailty patients within their own homes and across the acute site. The ANP will use expertknowledge and advanced clinical reasoning skills to provide specialist supportand a comprehensive geriatric assessment to aid decision making and plans forongoing management. The ANP will supportadmission avoidance and support the Hospital@Home model. The post holder will bepart of a larger multi-disciplinary team including Consultant Geriatricians,ANPs and Therapists. This post wouldsuit an experienced nurse who has an understanding of the management of peopleliving with frailty. Thisis a rotational across frailty services. Main duties of the job Thesuccessful candidate will provide leadership, clinical expertise andorganisation of the new Frailty H@H Service. They will continuouslydevelop and enhance knowledge and skills that will directly influence consistentstandards of clinical practice and ensure the highest possible standards ofevidence based care are achieved in order to promote patient focused care. They will promote patientcentred high quality evidence based care to patients on the ward, ensuring thepatient is central to all aspects of treatment and care. Communicate effectivelywith all the multi- disciplinary teams both internal and external to theorganisation. Provideclinical leadership to a group of staff within the H@H team and throughout thecare group. Full Job Descriptions can be found in the adverts supporting documents, The Frailty Advanced NursePractitioner will be experienced in the management of frail patients supportingadmission avoidance and safe, prompt return to the community. You will have expert knowledge and advanced clinicalreasoning skills to provide specialist support and advice to the EmergencyDepartment team and other healthcare staff and to manage a case load ofpatients within the Frailty Assessment Unit where the length of stay will beless than 72hrs. Assess, diagnose, treat and discharge adult patientsensuring appropriate advice, health education and referral to appropriatecommunity services. To perform autonomous diagnostic and prognosticinvestigations To make autonomous decisions regarding admitting ordischarging of patients within ED or the Frailty Assessment Unit using advancedclinical knowledge to underpin decisions. Work independently, with a high degree of autonomy, andtake part in clinical supervision from a named consultant. Be able to manage your own caseload in response tounpredictable patterns of work. Acceptreferrals from other health care professionals and work unsupervised. To clinically assess patients, prioritising clinical needand highlighting those patients who require immediate medical intervention. To act as a resource for all members of themulti-disciplinary team, providing advice concerning aspects of the managementof frailty. Support the ongoing development of the Frailty Team andUnit with an integrated approach to the delivery of acute medicine andunscheduled care involving all relevant stakeholders including health andsocial care organisations. The post requires a flexibleapproach to working and requires the post holder to work 8am-8pm covering a 7day service.
- Experience working in Frailty, Registered nurse qualification Appropriate degree level qualification at
- Masters level or willing to work towards
- Substantial experience at band 6 in an acute medical setting

- Substantial specialist experience of frail patients;
