Full Job Description

An opportunity has arisen to recruit a qualified ACP Band 8a Advanced Clinical Practitioner within the North Shropshire CMHS.

We are seeking a dynamic, highly motivated ACP who ideally has experience of working within a community setting. North Shropshire CMHS is a fully staffed, multi-disciplinary service providing assessment, brief intervention and specialist support to people experiencing severe & enduring care.

As a registered professional, you will be committed to providing a high quality service and will support the development of our CMHS whilst maintaining a high standard of patient care.,

  • The direct provision of high quality evidenced based person centred mental health care to service users within North Shropshire locality.

  • Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary service.

  • Facilitating learning through the development and delivery of specialist clinically focused education and training programmes.

  • Receive patients with both diagnosed and undiagnosed difficulties and make an assessment of their health care needs, based upon your highly developed nursing knowledge and skills which includes physical examinations skills.

  • Plan and coordinate the physical healthcare needs of service users in the community service: including the requirements for annual physical health examinations and investigations.

  • Order necessary investigations and provide treatment and care both individually, as part of a team and through consultation with / referral to other agencies e.g ECGs, Clozaril bloods etc.
