Full Job Description

We are looking for enthusiastic and dynamic Advanced Nurse Practitioners to join the new innovative specialist Hospital@Home Frailty Team/ Frailty Assessment Unit. We are in the process of developing a new pathway which involves the identification and management of frailty patients within their own homes and across the acute site.

The ANP will use expert knowledge and advanced clinical reasoning skills to provide specialist support and a comprehensive geriatric assessment to aid decision making and plans for ongoing management. The ANP will support admission avoidance and support the Hospital@Home model.

The post holder will be part of a larger multi-disciplinary team including Consultant Geriatricians, ANPs and Therapists. This post would suit an experienced nurse who has an understanding of the management of people living with frailty., The successful candidate will provide leadership, clinical expertise and organisation of the new Frailty H@H Service.

They will continuously develop and enhance knowledge and skills that will directly influence consistent standards of clinical practice and ensure the highest possible standards of evidence based care are achieved in order to promote patient focused care.

They will promote patient centred high quality evidence based care to patients on the ward, ensuring the patient is central to all aspects of treatment and care.

Communicate effectively with all the multi- disciplinary teams both internal and external to the organisation.

Provide clinical leadership to a group of staff within the H@H team and throughout the care group.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
