Full Job Description

We are looking for an experienced pharmacist to work into and provide pharmacy leadership within the adult community treatment teams in Gateshead. You will also be a part of the wider pharmacy team in the provision of mental health services within a range of specialities in the Trust.

We provide pharmacy and medicines optimisation services to all our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community mental health teams, crisis teams, psychiatric liaison and a variety of other specialist areas. Pharmacy staff are regarded as valued members of the multi-disciplinary team. We have several pharmacist independent prescribers, who are active within a range of clinical services.

Main duties of the job

Provide expert pharmaceutical input into the Adult CTTs in Gateshead (East and West) .

Receive enquiries and collaborate with medical and other clinical team staff on an individual-patient basis to provide specialist pharmaceutical input and appropriate treatment advice.

Develop and operate a medicine enquiry referral system.

Attend and contribute to CTT Team meetings, where appropriate.

Support the team with shared care and formulary prescribing issues within primary and secondary care including assisting with effective transfer of patients.

Advise on medication-related physical health requirements of patients.

Triage medication-related queries and referrals from primary care and provide advice where appropriate.

Effectively communicate medicines optimisation concerns to colleagues within the clinical teams

Support monitoring and management of side effects and high-risk medicines within the Adult CTTs.

Support shared decision making with patients, carers and families.

Support an open and reporting culture regarding medicine incidents and assist the CTT with their investigation and required actions, including After Action Reviews (AARs)., Develop, participate and assist in the co-ordination of local and national audits regarding psychotropic medicines within the CTTs.

Provide education and training sessions to CTT staff and Primary Care.

Support the implementation of local and national guidelines and policy e.g. NICE, CNTW policies

Develop role within the service in conjunction with CTT leads, pharmacy managers and advise on service developments.

Support community based clinical pharmacy technicians and direct work where appropriate.

Independent prescribing on an agreed individual basis, dependent on knowledge, skills and competence of post holder and the needs of the service.
