Full Job Description

To provide professional leadership to the team (in line with service philosophy), acting as a role model and demonstrating up to date knowledge of respiratory health issues and awareness of physical health needs, through the development of professional expertise and the maintenance of professional networks. 2. To promote the mental and physical well-being of Service Users by ensuring health promotion, psychological, rehabilitation and educational needs are addressed 3. To lead and coordinate the work of Care Teams, ensuring that their activities encompass all facets of Service Users care and are achieved in line with all Trust and local policies and procedures. 4. To actively seek the view of Service Users and Carers in the development and monitoring of services, ensuring high standards of care delivery. 5. To ensure that Service Users' care plans are developed to meet the assessed needs of the individual under the guidelines of Effective Care Coordination, including effective
care pathways for Service Users and Carers. 6. To be a point of contact for Service Users and families if they have any concerns about their care, to listen to their concerns and employ highly developed communication skills, to offer reassurance or negotiate solution.
