Full Job Description

This post is open to dynamic nurses or physiotherapists who would like to work in an exciting newly developed service providing community respiratory care for patients with a variety of chronic respiratory disease and respiratory failure in the Harrow catchment area. The post holder will join a flexible multi-professional team of nurses, physiotherapists, rehabilitation assistant and clinical administrator to provide, respiratory complex patient Case Management, Pulmonary Rehabilitation, Oxygen and Primary Care Education in line with national guidelines and best practice, to patients in their own homes, virtually or in clinic settings. The post holder will be required to work flexibly to provide a 9am - 5 pm weekday service in the community health care settings in Harrow, and across the Trust., Interview arrangements will be communicated via email so please check your email regularly following the closing date.
