Full Job Description

This newly created post is an exciting development as Cardiff and Vale UHBCardiff and Vale UHB continues to progress its enhanced community and intermediate care model delivering more care at home and less in hospital.This senior role will provide strategic leadership to Therapy Services working withstakeholders to shape and deliver integrated community services andinfluencing interdisciplinary models of care across therapies aligned with the Cardiff and Vale Rehabilitation Model. The postholder will join our creative Therapies leadership team which is committed to working together to deliver high quality, integrated person centred care supporting people to live well and enabling our teams to thrive., This post will lead Therapy Services within the Enhanced Community and Intermediate Care model. It will support the delivery of high-quality Therapy Services to deliver more care at home and less in hospital through interventions which preventadmission and support timely discharge. The post holder will provide strategic leadership, maximising the contribution of Therapies in community and intermediate care in providing effective integrated services to meet the needs of the population of Cardiff and the Vale.The postholder will work collaboratively with a range of stakeholders ensuring therapy services are fully integrated into the model and resources are used most effectively acrosscommunity and intermediate care. This is a developing model and the ability to horizon scan, articulate a strategic vision, create opportunities and lead change bybringing colleagueswith divergent priorities togetherwill be essential. As a new role the scope will develop as Therapies align services against

the organisational structure which this new post provides. The postholder will have staff and resource management responsibilities.
