Full Job Description

The post holder will lead the Children's Continuing Care Team and play an integral role within the AACHC team at the ICB. The successful candidate will be a senior clinician with significant experience of complex children's care. They will act as line manager for a small team. Working together with other professionals within the ICB and across the system, in both the public and private sector, the post holder will ensure a 'team around the child' ethos is developed and maintained for all children/young people who require support for their complex needs and are CCC eligible., The post holder will be responsible for providing management and clinical leadership for a team of healthcare professionals in the delivery of NHS Children and Young People's Continuing Care across Birmingham and Solihull ICB.

Support a range of children's commissioning activities, within the function of the All Age Continuing Healthcare Team.

Have operational oversight of the delivery of the NHS Children and Young People's Continuing Care function including assessments, reviews, commissioning and case management.

To identify service priorities, setting and monitoring standards, child centred care planning, and ensuring the team has the right skills and support to deliver a high quality service.

To communicate with, and actively influence, a range of internal and external stakeholders including clinical and professional leads, senior managers and executive board members to improve outcomes for this vulnerable group of children and young people.

The All Age Continuing Healthcare (AACHC)Team at Birmingham and Solihull ICB are looking to recruit a compassionate, experienced, highly motivated Children's Nurse with excellent communication and organisational skills as the Manager of Continuing Care (CCC) for Children and Young People.
