Full Job Description

Act as a first point of assistance for crew enquiries relating to patient journeys. Coordinate crews and patient collection/drop-off, making plans to pre-empt problems and solve issues when they arise. Act as a reliable point of contact for the UHBW Patient Transport Team in relation to incoming and outgoing patients., Communication:

  • Act as a first point of assistance for crew enquiries relating to patient journeys.
  • Facilitate and support effective communication between the UHBW Patient Transport Team, patient transport crews and other stakeholders to ensure smooth discharge/transfer of patient.
  • Patient Care :
  • Be present at the discharging ward to check details of patient's journey with particular focus on patient's mobility, details of access at their property and any relevant medical conditions and communicate any discrepancies or updates to the responding ambulance crew
  • Organisation and Planning:
  • Have a well-developed understanding of ward and clinic processes particularly those processes involved when discharging patients., people and vulnerable adults.
