Full Job Description

The Oxfordshire Children's Diabetes Service covers all areas served by the Children's Hospital, Oxford and the Horton Hospital, Banbury. There are approximately 370 children/young people in the service, mostly with Type 1 diabetes. There are an increasing number of CYP with Type 2 diabetes, Cystic Fibrosis Diabetes and other, rarer types. The service has an ethos of continual improvement, research and embracing technological advances in care to provide the best possible outcomes for all CYP. The nursing team are part of a multi-disciplinary team of dietitians, psychologists, consultants, specialty doctors, and support staff who work closely together. The lead nurse role is crucial in being a senior lead for this team and in relationships with the adult diabetes team for smooth transition of services for young people., The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care. · To lead and develop the specialist nursing service for children and young people (CYP) with diabetes and their families in line with Best Practice Tariff · To proactively case manage and anticipate the future care needs of people at risk and develop appropriate management plans accordingly. · To provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of the client group. · To monitor and lead improvements to standards of care through supervision of practice, clinical audit (including the NPDA), evidence-based practice, quality improvement (QI) programmes, teaching, education and supporting professional colleagues and the provision of skilled professional leadership. · To lead on the implementation of relevant research into practice contributing to the further development of evidence based research and audit. To contribute to clinical governance by leading audit and research as part of the MDT within the Paediatric Diabetes Service. · To develop robust partnerships within primary and secondary care and schools to ensure true integrated care for CYP Post holders will be expected to participate fully in their personal development and review process., The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care and maintain own CPD. · To lead and develop the specialist nursing service for children and young people (CYP) with diabetes and their families in line with Best Practice Tariff · To proactively case manage and anticipate the future care needs of people at risk and develop appropriate management plans accordingly. · To provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of the client group. · To monitor and lead improvements to standards of care through supervision of practice, clinical audit (including the NPDA), evidence-based practice, quality improvement (QI) programmes, teaching, education and supporting professional colleagues and the provision of skilled professional leadership. · To lead on the implementation of relevant research into practice contributing to the further development of evidence based research and audit. To contribute to clinical governance by leading audit and research as part of the MDT within the Paediatric Diabetes Service. · To develop robust partnerships within primary and secondary care and schools to ensure true integrated care for CYP Leadership & Management: · Role model standards of care and behaviour through clinical practice. · Be at the forefront of innovative practice, inspiring colleagues to implement such practice and service developments and encourage others to think of innovative practice also. · As part of the senior management team, contribute and lead the development of MDT within the service, building and sustaining relationships to maintain continuity and team message. · Maintain effective communication systems within the team, the directorate, the Trust and the CCG's. When required, represent the team in meetings and forums. · Contribute to local (Thames Valley) and national clinical networks and speciality specific groups, working in partnership with other key professionals. · Work in partnership with education establishments · Analyse relevant National policy, within area of expertise, advise on impact for the service, leading the implementation of changes and evaluation of processes in partnership with the MDT. · Participate and lead in local and national audits to ensure compliance with the national audit (NPDA). · Participate and lead quality improvement projects locally and nationally. · Maintain professional registration and practice through CPD. · Maintain professional portfolio to demonstrate competency in line with Trust policy · Maintain personal standards of conduct and behaviour consistent with Trust and NMC guidelines. · Allocate, coordinate, monitor and assess own workload and that of the diabetes nursing team, ensuring resources are deployed appropriately. · Facilitate and support the performance management of the team members, enabling regular appraisal, personal education and development planning, and undertaking revalidation. · Ensure appropriate use of resources in order to meet service/clients needs. · Ensure the service has adequate skilled staff and recruit when required. · Identify gaps in service provision and work with the MDT to implement the changes required to improve service provision. · Recognise and act as an advocate for patients, carers, service and organisation. · Lead change as required and ensure resolution of conflict. Clinical Practice: · Have overall nursing responsibility and accountability for a defined diabetes specialist caseload and support other specialist nurses as required. · Assess diagnosis, plan and implement programmes of evidenced based care for client group autonomously and with the team message. · Contribute to the coordination and effective management of the admission and discharge process with confidence and expertise, supporting inpatient nursing staff. · Ensure interventions are undertaken in line with best practice guidance for diabetes care in CYP. · Prescribe or dose adjust in accordance with Trust policy and expertise. · Undertake advanced skills appropriate to role, ensuring clinical competencies are maintained in accordance with the Trust policy. · Ensure written guidelines are available to all staff undertaking clinical investigations and care, eg insulin pumps, blood glucose checking, pen injection therapy, sick day management. · Provide advanced knowledge advice and support to client group and other professionals in hospital or in the community · Regularly attend and provide specialist input within the diabetes clinics at the Children's Hospital (JR), the Horton Hospital and Transition clinic at the Churchill Hospital as demanded by the service. As team lead, ensure all clinics are staffed by adequate number and skill mix of nursing team members. · Undertake nurse led clinics and virtual clinics. · Lead, co-ordinate and undertake school, nursery and college visits for CYP following diagnosis and annually as required by education authorities and best practice · Ensure that child safeguarding and vulnerable adult legislation is adhered to within own practice and service. · Take part in MDT meetings where a CYP is registered on the CP register / LAC / Child in Need / TAF. · Lead on the development and evaluate policies/guidelines/protocols for advanced and specialist clinical practice and take responsibility for updating and monitoring policies/guidelines/protocols specific area of expertise. · Prepare accurate and concise reports both verbally and written. · Communicate the needs of the client groups effectively within the MDT and the Trust. · Continually review and evaluate own role and clinical practice. Incorporate advanced practice, in line with the framework of the Trust and NMC. · Be an active member of relevant Trust committees and groups contributing to developing/implementing local/national strategy. · Plan and manage own workload and the workload of the service, in a flexible manner, ensuring that service provision meets the needs of clients. · Keep relevant records and submit reports in a professionally acceptable manner. · Contribute to the clinical governance process, including adverse incident reporting/investigation and the management of complaints, as appropriate. · Collect and collate data as required by the area of expertise in order to inform internal and external professional and operational bodies. · Be involved in peer review of own practice and that of the service., · Develop and deliver specific and relevant specialist teaching programmes for all disciplines and grades of staff, client group and carers within the Trust in relation to CYP diabetes. · Ensure clinical environment is conducive to supporting the education and learning of all staff and students. · Review and evaluate all diabetes CYP teaching programmes and lead on the development of new programmes for student nurses at Oxford Brookes University. Provide training opportunities/spoke placements. · Act as mentor/preceptor and resource person for all nursing and support staff, as appropriate to the role. · Develop and distribute educational leaflets in relevant formats, for staff and patients relating to service and patient pathway along with MDT · Identify own development needs in line with service requirements within personal development plan. · Ensure personal, peer support and clinical supervision needs are met. · Maintain education and networking links with the Thames Valley Diabetes Network, attending meetings at least once in a year. · As agreed within the Diabetes MDT, attend national and international conferences, presenting research, audit, programme developments to peers and learning from them. Support other specialist diabetes nurses in the team to also attend on a rotational basis. · Provide an environment that encourages client centred involvement where clients are facilitated to ask for help, advice and education. Research & Development: · Evaluate clinical practise through audit of service, developing and managing strategies to address any shortfalls, whether the national audit (NPDA) or local audits. · Demonstrate knowledge of current research in all aspects of his/her work and to advise others on the implementation of relevant research findings. · Lead the development of research proposals with the diabetes specialist nursing team, in communication with the research Lead Clinician and the Diabetes Research Team. Attend bi-monthly research meetings · Disseminate results of audit findings via local team and/or national/international meetings and publications. · Implement research findings appropriate to practice. · Promote the shared objectives of the multidisciplinary team by working closely to ensure that best practice is achieved.

The post holder must hold a Masters' degree in a subject relevant to the specialty. While this does not have to be a full diabetes masters, the post holder must have undertaken one degree/masters degree level module in Diabetes.,
