Full Job Description

Lakenham Surgery is currently seeking a qualified ANP or Nurse Practitioner to join our friendly and supportive GP, Nursing and Practice team. This key role aligns closely with both the GP and Nursing Teams, primarily supplementing practice capacity for On the Day and short-term pre-bookable appointments but also seeing and treating patients with long term conditions and a range of other presentations as required. The successful candidate will provide care that encompasses history taking, holistic clinical assessment, diagnosis, treatment and evaluation of care, demonstrating safe clinical decision making and awareness of professional competencies and boundaries at all times. Ideally, they will see, assess and treat all patients ranging from young children to elderly members of our patient population and have experience of dealing with all types of medical conditions, but recognise the need, where appropriate, to consult with or refer onto colleagues. Dealing with referrals, prescriptions
and test results for episodes of care provided will also be required.,

  • Manage individual patient consultations

  • Help patients to identify their health needs

  • To assess and diagnose across a wide range of health care conditions, order investigations, plan and provide treatment as needed and discharge patients, where appropriate, without reference to a GP

  • To recognise patients with more complex problems who do need the expertise of a GP and liaise appropriately with a member of the GP team, initiating appropriate and timely consultation and or referral, when a condition exceeds the Nurse Practitioners scope of practice and/or expertise

  • To demonstrate critical thinking and diagnostic reasoning skills in clinical decision making

  • To assess, tolerate and manage risk and deal with uncertainty in accordance with the context and requirements of the role

  • To prescribe in accordance with regulations and with the post holders training and registration

  • To prescribe safe, evidence based, cost-effective and appropriate medication as defined by current legislative framework and in accordance with local non-medical prescribing policy

  • To follow agreed national and local treatment guidelines, ensuring all care is delivered within the professional scope of practice.

  • To have clinical and professional accountability for prescribing practice, work at all times within clinical competence and with reference to the NMC professional standards.

    Previous experience in a GP Practice is essential, together with minor illness and injury training. The successful applicant will need excellent interpersonal and communication skills and we are looking for a really good team player who is able to provide support and help to colleagues to ensure that we provide the best possible care to our patients, in a mutually supportive environment.

    We are a long established surgery based just outside the centre of Norwich with approximately 8,900 patients. We aim to provide positive access to services and high standards of clinical care to our diverse patient population and are expanding our team team to further support these objectives.
