Full Job Description

A great opportunity has arisen for a Band 5 registered nurse to join our friendly, dynamic and motivated team in the Anticoagulation service at Withybush General Hospital. The applicant will need to be enthusiastic, adaptable and have good attention to detail. The successful candidate will assist in the management and organisation of the anticoagulation patients under the Health Boards care. The service manages the anticoagulation of patients with a number of conditions such as, Atrial Fibrillation, Deep Vein Thrombosis, Pulmonary Embolism, and other Thrombotic events, The successful applicant will :-

  • under the supervision of the Anticoagulation CNS, accept referrals for Anticoagulation service.
  • utilise and adhere to health board policies, written guidelines, and procedures with particular awareness to anticoagulation guidelines.
  • liaise closely with patients, carers/relatives, and members of the multidisciplinary team (MDT) ensuring their involvement with treatment assessment, planning and delivery of care.
  • provide patients and carers/relatives with clear, up to date verbal and written anticoagulation education.
  • participate in nurse led anticoagulation clinics, referring to Anticoagulation CNS as appropriate.
  • perform Point of Care Testing (POCT) on patients that are difficult to obtain venous blood samples from, following POCT training procedure. You will accurately document INR results.
  • follow up Direct Oral Anticoagulation (DOAC) patients at timely intervals with regards to compliance, adverse drug effects, patient concerns.
  • assist in the training or supervision of new staff and student nurses.

    The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
