Full Job Description

Due to the changes in the ARRS Scheme to introduce General Practitioners we are looking to expand our team by seeking to employ a newly qualified General Practitioner to work alongside our practices and PCN staff., In accordance with the Extended Access and Enhanced Access Contracts, as agreed, the post-holder will make him/herself available to undertake a variety of duties including surgery consultations, telephone consultations and queries.

  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred to by other health care workers.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current National and Local disease management protocols, developing care plans for health.
  • Providing relevant and appropriate health education. Referring to other care providers as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Complying with QOF requirements.

  • Sutton Primary Care Networks Our PCN staff currently includes Clinical Pharmacists, Physicians Associates, Social Prescribers, Paramedics, Physiotherapists, Care Coordinators, Nurse Associates, Mental Health Practitioners, Health and Wellbeing Coaches, Social Prescribers and Pharmacist Technicians.
