Full Job Description

If you are looking for a new challenge and really want to do to a job that seriously makes a difference to some of the most vulnerable residents of Telford & Wrekin, come and join our Social Care Brokerage Team. The Brokerage Team works as part of the Commissioning and Procurement Team in Adult Social Care Team to source care from contracted care providers to support physical and personal day to day needs, psychological support, specialist accommodation, home from hospital support, and end of life care support for vulnerable adults in our Borough. The work involves considerable use of IT case management systems supported by clear verbal and written communication Key requirements We have a hybrid approach to working with three days in office and two days working from homes. We also run 7 day working so a rota is in place for occasional weekend and bank holiday working to meet demand as required.
