Full Job Description

If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank.

To assess, plan, deliver and evaluate care for service users who are referred to the Assertive Outreach Service/Pathway, including the assessment and management of risk.

To be responsible for a caseload of patients as the named key worker/care coordinator/lead clinician (or other equivalent title)

To work alongside the service user in meeting the practical needs of everyday life such as activities of daily living, budgeting skills, social structures, leisure activities, etc. in a collaborative manner

We are looking to appoint a dynamic and suitably experienced Mental Health Practitioner, who is flexible and able to work on own initiative, as well as part of a Multi-Disciplinary Team.

The assertive outreach mental health practitioner will be embedded in the core community mental health teams. Their role will be to deliver mental health support and treatment to the cohort of service-users who experience complex psychosis and are identified as requiring assertive outreach in North East Essex.

The assertive outreach mental health practitioner will undertake co-produced, personalised care planning with service users where goals and interventions are based on each individual's strengths, assets and a sense of what will offer them a more meaningful life. This may include supporting service users to be 'gainfully' engaged as per their goals in education, employment, volunteering, training, recovery colleges, or other meaningful occupation, and should include relevant collaborative approach.
