Full Job Description

The post holder is responsible for the Assessment pathway within Poplar Ward at Rochford Hospital. The post holder will be responsible for caring for young people aged 13-18 requiring a short assessment admission admitted via A&E or community mental health services. The post holder will also be responsible for supporting the discharge pathway for the young people admitted to the ward to ensure smooth transition back to community services. The post holder will assist support of people with mental health needs and challenging behaviours, such as depression or psychosis, and working with young people who have likely felt or attempted suicidal. You will be required to provide person centred care and working, whilst supporting qualified staff to ensure the highest standards of care are upheld. Young people, relatives, carers and other staff will find the post holder flexible, approachable and with a helpful and friendly disposition, upholding the ethos of EPUT of good customer care at all times with an awareness of own attitude at all times.,

  • To undertake clinical mental health assessments and support the management of Children and Young People presenting in crisis.
  • Use clinical knowledge and expertise to formulate an assessment of relapse risk of the patient after specialist assessment. Liaise with other agencies caring for patients, including community mental health teams and GPs and use collateral information from relatives, GPs and past clinical reports, where available and appropriate, to provide care plans for all patients.
  • Being responsible for the development, planning and implementation of brief care and treatment interventions for individual children and young people.
  • Undertake clinical risk assessments, based on the latest empirical evidence and in compliance with local policies and procedures.
  • Act as the advocate for patients and relatives.
  • To be aware of own personal development needs and participate in regular supervision and appraisal processes.
  • Monitor and evaluate the standards of care and take actions to ensure that a high standard of care is maintained. Contribute to Audit to ensure quality standards are met.
  • If there is not a patient utilising the Admission pathway you may be required to support the inpatient ward in nursing duties.
  • You may be required to provide training and presentations to local providers on CAMHS care and preventing admission.
