Full Job Description

We're looking for a exceptional people to join Age UK Doncaster in our mission to take the fear out of ageing. As an Assessment officer you will work collaboratively across services and projects to ensure consistent and holistic assessments and reviews of people accessing Age UK Doncaster's activities using a strengths-based approach. As well as continually developing our approach in line with Local and National legislation, guidelines, and practices. You will work across the organisation to conduct assessments and reviews with people accessing our services and projects. You will work closely with the Client Journey Manager to continually develop the approach we use in line with the agreed policies and procedures of Age UK Doncaster. This will promote the dignity and independence of the people we support. Main Duties include:

  • Carry out initial holistic strengths-based assessment for people accessing our services and projects.
  • Co-produce appropriate and proportionate support plans
  • Supported signposting for individuals internally and to external support where appropriate
  • Accurately record information and keep all systems up to date
  • Liaise with the people we support, their important individuals and next of kin as required
  • Liaise with statutory agencies as required and maintain professional relationships
  • Support colleagues to evolve, develop and embed new approach's and ways of working where required, including taking part in projects and learning opportunities
  • Achieving agreed targets, reporting on key performance indicators and highlighting impact using case studies and life stories.
  • Work closely with the wider team to ensure appropriate cover for all services as per Age UK Doncaster continuity plan.
  • To undertake any other reasonable duties in line with the job purpose that may from time to time be required by management., This post is 35 hours per week Monday to Friday. 2.5FTE posts available, part time hours will be considered for all posts
