Are you an enthusiastic and dynamic individual ready to make a difference in the fast-paced world of Acute Medicine (AM)? We are seeking an exceptional individual to join our team and become an integral part of our patient-centred approach to care.
Acute Medicine has moved to a new model that consists of a Rapid Assessment Treatment Zone (RATZ), Assessment Unit (AU), Medical Short Stay Unit and the Older Persons' Short Stay Unit.
The AU is a dynamic and busy unit within the Emergency and Acute Directorate that provides essential assessment and treatment for acutely unwell patients within the Cardiff and Vale area. The AU provides care for patients under a variety of medical specialties.
Additionally, the AM model directly links with the Medical Same Day Emergency Care (MSDEC). MSDEC is an acute assessment and diagnostic area for ambulatory patients that is supported by a variety of clinicians.
The department is very well supported and has a number of staff that are highly knowledgeable to help your transition onto the ward, smooth.
As a PCN on AU you will be at the heart of facilitating flow through the department, ensuring the patient receives the best possible experience. You will be pivotal in ensuring that patients are having all the diagnostics, pharmacy and additional discharge requirements met to facilitate a smooth and effective discharge back home.
Informal visits welcome!, The primary role of the Patient Care Navigator is to support and patient flow within the Emergency and Acute Medicine Directorate.
The post holder will work alongside the ward sister/charge nurse of the Assessment Unit ensuring that all actions have been met to facilitate a safe discharge and progress patient care. This may include ensuring transport arrangements are in place, ensuring that family members are aware of plans, facilitating investigations and medications, including running and delivering forms and medication. Futhermore, ensure checklists are completed, prompting medical staff and other allied professionals about actions required for potential discharges.
We are seeking a highly organised individuals, who work extremely well under pressure with excellent communication skills.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
