Full Job Description

We are looking to recruit to an already existing team with a permanent team member. To help us fill an exciting role within the Hospital to Home team. The role will be working out of our Leamington HQ hub.

Please be aware there is a consultation that will be under way whilst we carry out this recruitment, start and finish times will be part of that consultation, these will be discussed at interview and confirmed.

The role will be working from our Leamington Headquarters base, with an expectation that you will work from other bases as demand allows.

This is an excellent opportunity to work within a dynamic team contributing to and supporting all aspects of getting people home from hospital, in a safe manner, whilst settling them in their home to promote a healthy recovery after being in hospital. This supporting the NHS, Public Health and our Fire Service strategies in making communities safer within Warwickshire.

We are looking for a highly motivated individual who will assist in the delivery of Hospital to Home and Prevention activities; working collaboratively to drive change internally and further strengthen our Health partnerships as part of the PPR Strategy.

As part of the role you must be committed to Safeguarding Children and Vulnerable Adults whilst carrying out your work. You will also be required to undergo an enhanced DBS check. We would also expect you to uphold the principles of the Core Code of Ethics.

Key Requirements

  • Full Driving licence (Manual)

  • Knowledge and Understanding of Partnership working

  • Able to effectively plan and organise own workload to meet deadlines, prioritising to satisfy competing demands and urgent and important issues.

  • Expression of interest - Your expression of interest should be no more than 500 words and reference the Job Description and Person Specification to explain why you believe you are suitable to work within the team, you should also explain what skills and attributes you have, to make you successful in this role., The Fluency Duty is outlined in the Code of Practice on the English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
