If so, we have an exciting opportunity for you to join our dedicated team as an Assistant Navigator with the potential for development to Band 4 within 12 months. As an Assistant Navigator in our Mental Health and Social Inclusion Hub, you will play a crucial role in assessing mental health referrals primarily through telephone consultations. Using your exceptional judgment and empathy, you will determine the needs, risks, and urgency of response for each individual. By delivering safe and effective services, you will guide them towards the appropriate level of support.   • Provide appropriate assistance and support to people who contact the service and their carers. This will include supporting individuals to access health, social care, community services and mainstream community life
  • Complete an assessment with people while considering their financial and other resources, enable them to access support services which will assist with their health and social care needs.
  • Contribute to and monitor risk assessments and risk management plans, using a traffic light system in consultation with appropriately qualified team members in line with Trust Procedures, involving other professionals as required
  • A commitment to delivering a flexible service which operates 24 hours a day, 365 days a year
  • To supervise and assist the Assistant Access Navigators on the Access Team to ensure a quality service is provided.
