Full Job Description

This post attracts a New Hire Bonus of £1,500 (pro- rata for part time staff), please see attached terms and conditions.

To support the integrated 0-19 Healthy Child Service (HCS) in providing an innovative and high quality service to improve Health Outcomes for children, young people and their families.

To support the assessment and review of clinical needs.

To work without direct supervision to support clients as part of the Universal and Targeted aspects of the service provision.

To liaise with colleagues within the clinical team to assist in the co-ordination and development of the service.

To develop specialist interest areas in order to enrich the knowledge and skills set within the Norfolk Children & Young People's Health Service.

To assist the Clinical Team in the management and delivery of care to clients. Duties may include:

Contacts with families will be delivered in a variety of ways such as families homes and venue based development reviews at a variety of locations across the locality.

Building on existing assessment of families' needs and deliver evidenced based health advice without direct supervision.

Receiving referrals, triaging and using appropriate sign posting.

Working jointly with Health Visitor for Specialist families. This may include communicating complex information with other agencies such as social care staff regarding work HCS are delivering to the family.

Support to clients with more complex needs.

Completing Duty - reviewing waiting lists and booking mandated development reviews and running reports within system one.

Working jointly with outside agencies such as the Early Childhood and Family Support service, Nursery settings and Speech and Language.

To write clinical records in a concise, accurate and timely manner in line with Cambridgeshire Community Services record keeping policy.

To independently carry out specified assessment and intervention as planned and discussed with the HCP practitioner and using own initiative and experience to modify according to the clients needs.

To hold own caseload with support from Specialist Community Public Health Nurse (SCPHN) for those families with planned packages of care.

To contribute to the care of all clients, ensuring safety and dignity at all times.

To liaise as appropriate with members of 0-19 HCP locality team.

To liaise as appropriate with parents/carers and other health, social or voluntary agencies.

To communicate effectively with clients in complex situations.

To be actively involved in the communication of complex information regarding client's needs and any other useful information to the integrated 0-19 HCP locality teams.

To be aware at all times of the needs of client groups and to promote and encourage positive health choices.

To promote the use of digital resources on Just One Norfolk to enhance self-care skills for children, young people, and their families.

To confidently utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working.

To support other localities including the Single Point of Access to ensure that all relevant clinical skills are kept up to date, and to maintain effective working relationships and continuity of services.

GENERAL DUTIES:

To provide timely data on referrals and clinical activity.

To uphold all aspects of the service and organisational policies and procedures.

To take individual responsibility for any health and safety issues and undertake obligations under the health and safety act.

Ensure all equipment used is safe, clean and suitable standard according to policies.

To take individual responsibility to report incidents as well as any identified actual or potential risks either to you or the organisation or others.

To attend mandatory training.

To attend relevant training courses and to maintain up to date clinical knowledge and skills in order to perform in the role.

To participate in staff and service development through attendance at service meetings and in service training.

Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job.

SPECIFIC DUTIES:

Duties will include working independently in a variety of settings and at times dealing with emotional and distressing circumstances.

Undertaking independent decision making regarding specific referrals to improve outcomes for children, young people and families within Norfolk's 0-19 HCP Teams, seeking supervision where appropriate.

Working across professional groups to develop and maintain relevant knowledge and skills.

To assist in the prioritisation of referrals and booking appointments.

To engage in team and locality meetings.

To ensure responsibility for the health, safety and wellbeing of clients.

To identify unmet needs and gaps in service provision and highlight these to SCPHN or Team Leader.

To provide support and guidance and practical training to new members of staff.

CLINICAL AND PRACTICE GOVERNANCE:

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes.

The post holder must adhere to infection control policies and procedures.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and other audits as required.

Provide patients and their families/carers with information on standards they should expect from the team.

The post holder is required to participate in relevant emergency preparedness process for their team.
