Full Job Description

The Therapeutic intervention service delivers both group work and individualised work to people of all ages newly diagnosed with dementia and living in the community.

Integral to the service is the strong commitment to support families and carers given by the team family peer support workers.

The role of the assistant practitioner is patient facing and delivering group based educational programmes over an 8 week period at a variety of sites across the county., The role is primarily concerned with delivering group based educational information to people of all ages diagnosed with dementia across the county. This includes maintaining accurate records and supporting the wider team to safely manage the waiting lists.

This role requires excellent communication skills both digital and verbal and a confidant manner when speaking to the public.

You will be required to have some knowledge and experience of working with people diagnosed with dementia or other mental illness and be able to offer support and care to them whilst they are attending the group.

You need to be able to work effectively within a team whilst being supported by registered staff.
