Full Job Description

We are looking for anenthusiastic and motivated Assistant Practitioner to work in our fast paced andwell-established Front Door Integrated Discharge Team based at Queens MedicalCentre. We provide specialistservice within Nottingham University Hospitals Emergency Department and theAssessment Areas as well as working as an integral part of the Frailty Pathway. Our team ethos is toget our patients to the right place, at the right time, with the services thatthey need without a delay. The service is aimed atsupporting the Comprehensive Geriatric Assessment (CGA) as early as possible. Starting within the Emergency Department and working alongside aspecialist multidisciplinary team to determine the best management, treatment,and discharge plan for each patient. This will be workingclosely with Occupational therapists, Physiotherapists and Nurses within theservice, as well as Consultants, Doctors, Advanced Practitioners and a range ofHealth and Social Care Professionals. You will need excellentcommunication and interpersonal skills. You should be able to prioritise yourworkload, manage your time effectively, and work flexibly within the team. Youwill need to be dynamic and have a range of problem-solving skills. Working as part of an internal rotasystem between Monday Friday and 8am-6pm on weekends and bankholidays. Main duties of the job

  • Thepost holder will be working as a multi-disciplinary team member to undertakecomprehensive assessment and intervention to clients within the Pre-admissionareas of Nottingham University Hospitals Queens Medical Centre Campus in orderto facilitate the clients discharge or appropriate pathway identification.
  • Theywill need sound knowledge and experience of working with complex, frail andolder people that are able to manage a caseload that consists of a wide rangeof cases for people presenting multi pathologies, as well as challengingmedical and social circumstances.
  • Performholistic assessments and a range of skills within a competency framework, thepost holder will be expected to complete initial assessments and follow upsunder the guidance and supervision of the Clinical staff.
  • Utilisecommunity knowledge and skills whilst assessing and recommending appropriatedischarge support.
  • Haveexcellent assessment, communication skills and intervention skills to be ableto plan, develop and evaluate goal focussed rehabilitation.
  • Able to prioritiseworkload, manage their time effectively, and work flexibly within the team
  • They will have ongoing supervision from a clinical member of staffdirected and guided by your needs and be expected to participate in staffdevelopment programmes., To work autonomously and alongside Nurses, Physiotherapists, Occupational therapists and Assistant Practitioners as a multi-disciplinary team member, to undertake comprehensive assessment and intervention to clients within the pre-admission areas of Nottingham University Hospitals Queens Medical Centre Campus, in order to facilitate the clients discharge or appropriate pathway identification.

  • This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness.

  • To undertake individual treatment and interventions with clients under the supervision of qualified staff.
  • To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role (as per AP guidelines).
  • To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
  • To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.
  • To assess patients fully in line with their competency framework. This includes, obtaining full collateral histories, assessing all elements of wellbeing as outlined (including transfers, mobility, Mental Capacity), and determining appropriate plans from the information.
  • Demonstrate effective communication to provide instruction, advice and intervention to service users and carers in line with the multi-disciplinary team guidelines, (as per AP guidelines).
  • To assess, provide appropriate equipment and demonstrate use of a wide range of equipment to promote independence. (As per AP guidelines).
  • To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines (as per AP guidelines).
  • To attend and contribute to team meetings and case discussions using effective communication.
  • To maintain comprehensive and accurate records in line with polices
  • To undertake a range of nursing, physiotherapist, and occupational therapy competencies, as per the AP competency framework.

  • To participate in the development of good practice for the AP role within Nottingham CityCare Partnership
  • To provide support to other Assistant Practitioners as appropriate.
  • To be responsible for own Continuing Professional Development, incorporating it into your work as necessary.
  • To use information technology on a daily basis and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy. To always have regard for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.
  • To actively participate in the services training strategy, including in-service training, and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials, and training sessions.
  • To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations.
  • To always have regard for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.

  • To liaise effectively with other professionals and agencies., To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines).
  • To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio.

  • To keep up to date in basic IT skills in line with the modernisation of data collection records.
  • To participate in appropriate in-service training and individual learning with specific and measurable outcomes.

    Educated to Foundation degree or equivalent in a health-related discipline, Ability to be flexible over hours worked within contracted hours to meet the needs of the service.
