Full Job Description

Applications are sought from Allied Health Professional, Psychology and Nursing Candidates holding a suitable foundation degree level 4 or level 5, for an exciting opportunity to work within the Westbridge In-patient to Community Transition Team.
The successful candidate will be offered a substantive position, working 37.5 hrs per week. Based at Westbridge, but by its nature, this role will require travel between in-patient and community bases, excellent diary management, and good delegation skills.
The Team work with individuals who have mental health disorders or personality disorders, who have spent many years in hospital and are currently admitted to Secure Care Inpatient Services to enable a successful transition into the community.
The postholder will be embedded within in-patient and community multi-disciplinary teams and will contribute their observations of physical, communication and occupational barriers, and strengths of the individuals., Duties include providing direct care in assisting the preparation and implementation of assessments, treatments and discharge planning, delivering 1:1 and group treatments which will help individuals to develop the skills required to safely transition to life in the community.
The post-holder will have a demonstrable interest in the collaborative process of recovery, supporting the multi-disciplinary team, providing trauma informed care, assisting in a range of assessment-based interventions, in the development of detailed person-centred care-plans, focussing on supporting the individual in their occupational, functional, emotional and treatment needs.
There will be a commitment to reducing inequality and discrimination, and[INS: :INS] following training the postholder will recognise the many aspects of diversity including "protected characteristics" as defined under the Equality Act (2010) and the NHS Core20PLUS5., · Supporting qualified staff to complete a range of specialised assessments including in-patient referrals to Westbridge and the Community Teams, Mental Capacity Assessments, Care Act reviews and Financial Assessments.
· Form a professional judgement of an individual's health condition.
· Attending and contributing to CPA reviews, Professional's Meetings, and Discharge Planning Meetings.
· Submitting applications for benefits, supported accommodation, residential and nursing care placements.
· Liaising and engaging placement providers and external organisations as required.
· Direct patient care identifying and accessing supportive networks, occupation, leisure activities, community healthcare providers, substance misuse education and support providers.
· Delivering treatment and therapies to support individuals' transition and living meaningful lives in the community, for example, substance misuse work, well person and trauma informed care.
· Effective and timely communication with individuals and families as part of the discharge process.
· Communicate and complete records for individuals.
· Post discharge follow-up reviews.
